Genes affected
SUFU (HGNC:16466): (SUFU negative regulator of hedgehog signaling) The Hedgehog signaling pathway plays an important role in early human development. The pathway is a signaling cascade that plays a role in pattern formation and cellular proliferation during development. This gene encodes a negative regulator of the hedgehog signaling pathway. Defects in this gene are a cause of medulloblastoma. Alternative splicing results in multiple transcript variants.[provided by RefSeq, May 2010]

The SUFU c.1273G>A (p.Ala425Thr) variant has been reported in the published literature in families with hereditary breast and ovarian cancer syndrome, including one family that also carried a pathogenic BARD1 variant (PMID: 28050010 (2017), 27153395 (2016)). To the best of our knowledge, this variant has not been reported in individuals with SUFU-related disorders. The frequency of this variant in the general population, 0.000093 (12/129200 chromosomes in European (Non-Finnish) subpopulation (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is higher than would generally be expected for pathogenic variants in this gene. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is damaging. Based on the available information, we are unable to determine the clinical significance of this variant. -

In silico analysis supports that this missense variant does not alter protein structure/function; Observed in an individual with breast cancer who also harbored a pathogenic variant in the BARD1 gene in the published literature (Feliubadal et al., 2017); This variant is associated with the following publications: (PMID: 12426310, 28050010) -

This sequence change replaces alanine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 425 of the SUFU protein (p.Ala425Thr). This variant is present in population databases (rs748099523, gnomAD 0.009%). This missense change has been observed in individual(s) with breast cancer (PMID: 28050010). ClinVar contains an entry for this variant (Variation ID: 453958). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt SUFU protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Ambry Genetics
Significance:Likely benign
Review Status:criteria provided, single submitter
Collection Method:clinical testing

This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
