This variant has been reported in 5 individuals affected with familial hypercholesterolemia (PMID: 20809525, 30270082; ClinVar SCV000583671.1, SCV000947750.3) and in one individual suspected to be affected with familial hypercholesterolemia (PMID: 34037665). It has been shown that this variant segregates with disease in two affected individuals in one family (PMID: 30270082). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Based on the available evidence, this variant is classified as Likely Pathogenic. - |

This missense change has been observed in individuals with familial hypercholesterolemia (PMID: 20809525, 34037665; Invitae). ClinVar contains an entry for this variant (Variation ID: 251211). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt LDLR protein function with a positive predictive value of 95%. This variant disrupts the p.Asp139 amino acid residue in LDLR. Other variant(s) that disrupt this residue have been observed in individuals with LDLR-related conditions (PMID: 10634824, 20809525, 21418584, 22883975, 23375686), which suggests that this may be a clinically significant amino acid residue.
