The c.404delC pathogenic mutation, located in coding exon 3 of the FLCN gene, results from a deletion of one nucleotide at nucleotide position 404, causing a translational frameshift with a predicted alternate stop codon (p.P135Lfs*42). This variant, also described as c.402del, was reported in individual(s) with features consistent with Birt-Hogg-Dube syndrome (Namba Y et al. PLoS One. 2023 Jul;18:e0289175; Jikuya R et al. EBioMedicine. 2023 Jun;92:104596; Kumasaka T et al. Histopathology. 2014 Jul;65:100-10). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -
