Variant summary: KIF1B c.1456C>G (p.Pro486Ala) results in a non-conservative amino acid change located in the Kinesin-associated domain (IPR032405) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 0.00023 in 1595604 control chromosomes (i.e. in 362 carriers) in the gnomAD database (v4.1 dataset). The occurrence in several carriers suggests that this variant is likely not associated with a high penetrance, severe, early onset disease phenotype in heterozygous state. The variant, c.1456C>G has been reported in the literature in 2 individuals affected with suspected Charcot-Marie-Tooth Disease (Volodarsky_2021), however no supportive evidence for causality was provided. This report does not provide unequivocal conclusions about association of the variant with Charcot-Marie-Tooth disease type 2A1. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 32376792). ClinVar contains an entry for this variant (Variation ID: 476778). Based on the evidence outlined above, the variant was classified as uncertain significance. -

The KIF1B c.1456C>G variant is predicted to result in the amino acid substitution p.Pro486Ala. This variant has been reported in two individuals with Charcot-Marie-Tooth disease (Supplementary Table 2. Volodarsky et al. 2021. PubMed ID: 32376792). This variant is reported in 0.028% of alleles in individuals of Latino descent in gnomAD, which may be too common to be a primary cause of disease. This variant has conflicting interpretations in ClinVar ranging from likely benign to uncertain (https://www.ncbi.nlm.nih.gov/clinvar/variation/476778/). Although we suspect that this variant may be benign, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
