Variant summary
Our verdict is Likely pathogenic. The variant received 9 ACMG points: 9P and 0B. PVS1PP5
The NM_005076.5(CNTN2):c.1516C>T(p.Arg506*) variant causes a stop gained change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000868 in 1,613,804 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. R506R) has been classified as Benign. Variant results in nonsense mediated mRNA decay.

Variant summary: CNTN2 c.1516C>T (p.Arg506X) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. The variant allele was found at a frequency of 1.2e-05 in 247468 control chromosomes. To our knowledge, no occurrence of c.1516C>T in individuals affected with Epilepsy, Familial Adult Myoclonic, 5 and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 659979). Based on the evidence outlined above, the variant was classified as pathogenic.
This variant has not been reported in the literature in individuals affected with CNTN2-related conditions. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This sequence change creates a premature translational stop signal (p.Arg506*) in the CNTN2 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in CNTN2 are known to be pathogenic (PMID: 11178983, 23518707). ClinVar contains an entry for this variant (Variation ID: 659979). For these reasons, this variant has been classified as Pathogenic.
