Variant summary

Our verdict is Uncertain significance. Variant got 3 ACMG points: 3P and 0B. PM2PP3

The NM_206933.4(USH2A):ā€‹c.13776G>Cā€‹(p.Gln4592His) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000342 in 1,461,846 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/21 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (ā˜…ā˜…). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. Q4592R) has been classified as Likely benign.

Genes affected
USH2A (HGNC:12601): (usherin) This gene encodes a protein that contains laminin EGF motifs, a pentaxin domain, and many fibronectin type III motifs. The protein is found in the basement membrane, and may be important in development and homeostasis of the inner ear and retina. Mutations within this gene have been associated with Usher syndrome type IIa and retinitis pigmentosa. Multiple transcript variants encoding different isoforms have been found for this gene. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251212 control chromosomes (gnomAD). c.13776G>C has been reported in the literature in individuals affected with Usher Syndrome (e.g. Dreyer_2008, Dad_2016). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 18273898, 27957503). ClinVar contains an entry for this variant (Variation ID: 554572). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
