Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4BP6
The NM_206933.4(USH2A):c.13622A>C(p.Gln4541Pro) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000744 in 1,613,914 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 13/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Synonymous variant affecting the same amino acid position (i.e. Q4541Q) has been classified as Likely benign.

Inborn genetic diseases Uncertain:1
The c.13622A>C (p.Q4541P) alteration is located in exon 63 (coding exon 62) of the USH2A gene. This alteration results from a A to C substitution at nucleotide position 13622, causing the glutamine (Q) at amino acid position 4541 to be replaced by a proline (P). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
not provided Uncertain:1
This sequence change replaces glutamine, which is neutral and polar, with proline, which is neutral and non-polar, at codon 4541 of the USH2A protein (p.Gln4541Pro). This variant is present in population databases (rs143032902, gnomAD 0.003%). This variant has not been reported in the literature in individuals affected with USH2A-related conditions. ClinVar contains an entry for this variant (Variation ID: 48420). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt USH2A protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
not specified Benign:1
The Gln4541Pro variant in USH2A has not been reported in the literature nor prev iously identified by our laboratory. However, this residue is not highly conserv ed in mammals and computational analyses (PolyPhen2, SIFT, AlignGVGD) do not sug gest a high likelihood of impact to the protein. In addition, this variant was i dentified in a patient with a alternate clear explanation of disease. In summary , these observations suggest that this variant is likely benign. -
