The p.Arg801Leu variant in RYR2 has been identified by our laboratory in 1 Cauca sian adolescent with HCM. The variant has also been identified in 3/29120 Europe an chromosomes by the Exome Aggregation Consortium (ExAC, http://exac.broadinsti tute.org; dbSNP rs371157868). Computational prediction tools and conservation an alysis suggest that this variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, the clinical s ignificance of the p.Arg801Leu variant is uncertain.
Variant summary: RYR2 c.2402G>T (p.Arg801Leu) results in a non-conservative amino acid change located in the SPRY domain (IPR003877) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 1.3e-05 in 233128 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.2402G>T in individuals affected with Catecholaminergic Polymorphic Ventricular Tachycardia and no experimental evidence demonstrating its impact on protein function have been reported. Co-occurrences with other pathogenic variant(s) have been reported (MYBPC3 c.655G>C, p.Val219Leu, in an internal LCA sample; and this variant is classified as pathogenic by our lab for Hypertrophic Cardiomyopathy), providing supporting evidence for a benign role. Five clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation, and all of them classified the variant as uncertain significance.
