This sequence change replaces histidine, which is basic and polar, with arginine, which is basic and polar, at codon 402 of the FH protein (p.His402Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with clinical features of hereditary leiomyomatosis and renal cell cancer (external communication, internal data). ClinVar contains an entry for this variant (Variation ID: 265149). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt FH protein function with a positive predictive value of 95%. For these reasons, this variant has been classified as Pathogenic. -
The H402R variant in the FH gene has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. The H402R variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The H402R variant is a conservative amino acid substitution, which is not likely to impact secondary protein structure as these residues share similar properties. This substitution occurs at a position that is conserved across species, and in silico analysis predicts this variant is probably damaging to the protein structure/function. A missense variant in a nearby residue (G397R) has been reported in the Human Gene Mutation Database in association with multiple leiomyomatosis (Stenson et al., 2014), supporting the functional importance of this region of the protein. Based on the currently available evidence, it is unclear whether this variant is a pathogenic variant or a rare benign variant. We consider it to be a variant of uncertain significance. -

The p.H402R variant (also known as c.1205A>G), located in coding exon 8 of the FH gene, results from an A to G substitution at nucleotide position 1205. The histidine at codon 402 is replaced by arginine, an amino acid with highly similar properties. This alteration has been observed in multiple individuals with a personal and/or family history that is consistent with FH-related disease (Ambry internal data). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the majority of available evidence to date, this variant is likely to be pathogenic. -
