FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. The FH c.739-2A>G variant occurs in a canonical splice site (acceptor) and is therefore predicted to disrupt the normal gene product. A literature search was performed for the gene and cDNA change. This variant has not been reported in the germline, but was identified in a leiomyoma tumor sample (Mäkinen et al. 2017). The c.739-2A>G variant is not found in the Genome Aggregation Database in a region of good sequencing coverage so the variant is presumed to be rare. Based on the predicted disruption of the normal gene product and application of ACMG criteria, the c.739-2A>G variant is classified as likely pathogenic for hereditary leiomyomatosis and renal cell cancer.

This sequence change affects an acceptor splice site in intron 5 of the FH gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in FH are known to be pathogenic (PMID: 11865300, 21398687). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with leiomyomas, uterine fibroids and renal cell cancer (Invitae). ClinVar contains an entry for this variant (Variation ID: 654446). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic.

The c.739-2A>G intronic variant results from an A to G substitution two nucleotides upstream from coding exon 6 in the FH gene. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. Other variant(s) impacting the same acceptor site (c.739-1G>T) have been identified in individual(s) with features consistent with Hereditary leiomyomatosis and renal cell cancer (HLRCC) (Ambry internal data). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as likely pathogenic.
