FH (HGNC:3700): (fumarate hydratase) The protein encoded by this gene is an enzymatic component of the tricarboxylic acid (TCA) cycle, or Krebs cycle, and catalyzes the formation of L-malate from fumarate. It exists in both a cytosolic form and an N-terminal extended form, differing only in the translation start site used. The N-terminal extended form is targeted to the mitochondrion, where the removal of the extension generates the same form as in the cytoplasm. It is similar to some thermostable class II fumarases and functions as a homotetramer. Mutations in this gene can cause fumarase deficiency and lead to progressive encephalopathy. The FH c.655G>A (p.Asp219Asn) variant has been reported in the published literature in individuals with FH deficient uterine leiomyomas (PMID: 32612247 (2020)). Additionally, the variant has been detected in a dermatofibrosarcoma protuberans FFPE specimen, which also harbored a homozygous deletion of CDKN2A and TP53 frameshift variant (PMID: 25852058 (2015)). The frequency of this variant in the general population, 0.000054 (7/129016 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant. -
