This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 116 of the MUTYH protein (p.Arg116Trp). This variant is present in population databases (rs373766973, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with MUTYH-related conditions. ClinVar contains an entry for this variant (Variation ID: 142812). An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The tryptophan amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. Studies have shown that this missense change is associated with inconclusive levels of altered splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The MUTYH c.346C>T (p.Arg116Trp) variant has been reported in the published literature in individuals with breast cancer and reportedly healthy individuals (PMID: 33471991 (2021), see also LOVD (https://databases.lovd.nl/shared/variants/MUTYH)). The frequency of this variant in the general population, 0.000098 (3/30616 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Additional analysis using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant may affect proper MUTYH mRNA splicing. Based on the available information, we are unable to determine the clinical significance of this variant. -

Variant summary: MUTYH c.346C>T (p.Arg116Trp) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. Several computational tools predict a significant impact on normal splicing: Three predict the variant weakens a 5' donor site. One predict the variant has no significant impact on splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 6e-05 in 251474 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in MUTYH causing MUTYH-Associated Polyposis (6e-05 vs 0.0046), allowing no conclusion about variant significance. c.346C>T has been reported in the literature in at least two individuals affected with colorectal cancer, without strong evidence for causality (e.g. Pearlman_2021). This report does not provide unequivocal conclusions about association of the variant with MUTYH-Associated Polyposis. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The MYH p.Arg116Trp variant is a missense change in exon 3, and is conserved across mammals/species and computational analyses (PolyPhen, SIFT) suggest that this variant may impact the protein. The variant occurs in the last three bases of the exon. This position has been shown to be part of the splicing consensus sequence and variants involving this position sometimes affect splicing. A modest alteration in the splice predition was observed by one of the five prediction programs used. However, the above evidence is not predictive enough to assume pathogenicity or rule out pathogenicity. In summary, the clinical significance of this variant cannot be determined with certainty at this time. -
