Variant summary
Our verdict is Pathogenic. The variant received 13 ACMG points: 13P and 0B. PS1_ModeratePM1PM2PM5PP3_StrongPP5
The NM_001128425.2(MUTYH):āc.337T>Cā(p.Trp113Arg) variant causes a missense change. The variant allele was found at a frequency of 0.00000186 in 1,613,986 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Likely pathogenic in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W113G) has been classified as Likely pathogenic.

Hereditary cancer-predisposing syndrome Uncertain:2
The p.W113R variant (also known as c.337T>C), located in coding exon 3 of the MUTYH gene, results from a T to C substitution at nucleotide position 337. The tryptophan at codon 113 is replaced by arginine, an amino acid with dissimilar properties. In one study, this alteration was detected in conjunction with MUTYH p.Y179C alteration in two individuals reported to have polyposis (McVeigh TP et al. Ir Med J, 2016 Dec;109:485). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
This missense variant replaces tryptophan with arginine at codon 113 of the MUTYH protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported to co-occur with the pathogenic variant c.536A>G (p.Tyr179Cys) in 2 related individuals, one affected with early onset rectal cancer and the other with multiple polyps (PMID: 28644590). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Familial adenomatous polyposis 2 Uncertain:1
This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 113 of the MUTYH protein (p.Trp113Arg). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with colorectal cancer and/or polyps who also carried another pathogenic variant in MUTYH (PMID: 28644590). ClinVar contains an entry for this variant (Variation ID: 533303). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
