Variant summary
Our verdict is Likely pathogenic. Variant got 8 ACMG points: 8P and 0B. PM1PM2PP3_Strong
The NM_001048174.2(MUTYH):c.253T>A(p.Trp85Arg) variant causes a missense change. The variant allele was found at a frequency of 0.00000657 in 152,122 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another nucleotide change resulting in same amino acid change has been previously reported as Uncertain significancein ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W85G) has been classified as Uncertain significance.

Familial adenomatous polyposis 2 Uncertain:2
Uncertain significance, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Jan 04, 2024 | This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 113 of the MUTYH protein (p.Trp113Arg). This variant is present in population databases (rs776719541, gnomAD 0.002%). This missense change has been observed in individual(s) with rectal cancer and multiple polyps who also carried another pathogenic variant in MUTYH. (Invitae). ClinVar contains an entry for this variant (Variation ID: 230915). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. - |
Uncertain significance, criteria provided, single submitter | clinical testing | All of Us Research Program, National Institutes of Health | Aug 15, 2023 | This missense variant replaces tryptophan with arginine at codon 113 of the MUTYH protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). Splice site prediction tools suggest that this variant may not impact RNA splicing. To our knowledge, functional studies have not been performed for this variant. This variant has been reported in 1 individual affected with rectum cancer and polyposis who also carried another pathogenic variant in MUTYH, and another individual with polyposis in the same family (PMID 28644590). This variant has been identified in 2/282864 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |
Hereditary cancer-predisposing syndrome Uncertain:2
Uncertain significance, criteria provided, single submitter | clinical testing | Ambry Genetics | Mar 28, 2023 | The p.W113R variant (also known as c.337T>A), located in coding exon 3 of the MUTYH gene, results from a T to A substitution at nucleotide position 337. The tryptophan at codon 113 is replaced by arginine, an amino acid with dissimilar properties. A different alteration at the same nucleotide, c.337T>C, which results in the same amino acid change (p.W113R), has been reported in conjunction with a pathogenic MUTYH mutation (phase unknown) in two Irish polyposis patients (McVeigh TP et al. Ir Med J. 2016 Dec 12;109(10):485). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
Uncertain significance, criteria provided, single submitter | clinical testing | Color Diagnostics, LLC DBA Color Health | Jul 26, 2023 | This missense variant replaces tryptophan with arginine at codon 113 of the MUTYH protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been performed for this variant. This variant has been reported as biallelic (phase unknown) in 2 individuals from the same family affected with rectal cancer and polyposis and polyposis, respectively (PMID: 28644590). This variant has been identified in 2/282864 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. - |
