MUTYH (HGNC:7527): (mutY DNA glycosylase) This gene encodes a DNA glycosylase involved in oxidative DNA damage repair. The enzyme excises adenine bases from the DNA backbone at sites where adenine is inappropriately paired with guanine, cytosine, or 8-oxo-7,8-dihydroguanine, a major oxidatively damaged DNA lesion. The protein is localized to the nucleus and mitochondria. This gene product is thought to play a role in signaling apoptosis by the introduction of single-strand breaks following oxidative damage. Mutations in this gene result in heritable predisposition to colorectal cancer, termed MUTYH-associated polyposis (MAP). Multiple transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2017]

Another alteration at the same codon, p.M15V, has been reported as co-occurring with a pathogenic MUTYH mutation in three siblings with colorectal cancer in their forties and a history of multiple adenomas in their fifties and sixties (Segu&iacute; N et al. Gut. 2015 Feb;64:355-6). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -

This variant has not, to our knowledge, been published in the literature as pathogenic or benign. MUTYH Met15Thr was not observed at a significant allele frequency in large population cohorts (Lek 2016). This variant is located within the RPA binding domain (Ruggieri 2013). In silico analysis, which includes protein predictors and evolutionary conservation, supports a deleterious effect. Based on currently available evidence, it is unclear whether MUTYH Met15Thr is a pathogenic or benign variant. We consider it to be a variant of uncertain significance. Of note, MUTYH-Associated Polyposis (MAP) is a recessive condition associated with two MUTYH pathogenic variants on opposite chromosomes. -

In an alternate transcript (NM_001128425.1), this variant is also known as c.44T>C (p.Met15Thr). This variant was reported in an individual with breast cancer (Breast Cancer Association Consortium et al 2021. PubMed ID: 33471991). This variant has also been identified with second MUTYH pathogenic variant in a family with colorectal cancer (Seguí N et al 2014. PubMed ID: 24691292). In vitro functional studies showed that this variant disrupts the start codon of the two transcripts that encode the nuclear MUTYH isoforms, but does not result in complete loss of mRNA expression (Seguí N et al 2014. PubMed ID: 24691292). This variant is reported in 0.0058% of alleles in individuals of Latino descent in gnomAD and has conflicting interpretations regarding its pathogenicity in ClinVar, ranging from uncertain to pathogenic (https://www.ncbi.nlm.nih.gov/clinvar/variation/406849/). Although we suspect that this variant may be pathogenic, at this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
