Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt DOCK7 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

The c.842T>A (p.I281N) alteration is located in exon 8 (coding exon 8) of the DOCK7 gene. This alteration results from a T to A substitution at nucleotide position 842, causing the isoleucine (I) at amino acid position 281 to be replaced by an asparagine (N). Based on data from gnomAD, the A allele has an overall frequency of 0.007% (10/152224) total alleles studied. The highest observed frequency was 0.065% (10/15284) of Admixed American alleles. This alteration has been identified in the homozygous state in an individual with an abnormal EEG and intractable complex seizures (external communication 2024). This amino acid position is highly conserved in available vertebrate species. The in silico prediction for this alteration is inconclusive. Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
