The c.1346T>C (p.M449T) alteration is located in exon 13 (coding exon 12) of the PLEKHG5 gene. This alteration results from a T to C substitution at nucleotide position 1346, causing the methionine (M) at amino acid position 449 to be replaced by a threonine (T). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.

This sequence change replaces methionine with threonine at codon 449 of the PLEKHG5 protein (p.Met449Thr). The methionine residue is moderately conserved and there is a moderate physicochemical difference between methionine and threonine. This variant is not present in population databases (ExAC no frequency). This variant has not been reported in the literature in individuals affected with PLEKHG5-related conditions. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
