AGL (HGNC:321): (amylo-alpha-1, 6-glucosidase, 4-alpha-glucanotransferase) This gene encodes the glycogen debrancher enzyme which is involved in glycogen degradation. This enzyme has two independent catalytic activities which occur at different sites on the protein: a 4-alpha-glucotransferase activity and a amylo-1,6-glucosidase activity. Mutations in this gene are associated with glycogen storage disease although a wide range of enzymatic and clinical variability occurs which may be due to tissue-specific alternative splicing. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant allele was found at a frequency of 8e-06 in 250204 control chromosomes. c.3682C>T has been reported in the literature in individuals affected with Glycogen Storage Disease Type III (example, Shen_1996, Crushell_2010). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as pathogenic/likely pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -

The p.Arg1228Ter variant has been reported in at least three studies in which it is found in a total of six individuals with glycogen storage disease type III including in one in a homozygous state, in at least three in a compound heterozygous state and in two in a heterozygous state with no second allele described (Lucchiari et al. 2002; Shen et al. 1996; Crushell et al. 2010). The p.Arg1228Ter variant was absent from 20 controls and is reported at a frequency of 0.000058 in the East Asian population of the Genome Aggregation Database but this is based on one allele in a region of good sequence coverage so the variant is presumed to be rare. Based on the clinical evidence and the potential impact of stop-gained variants, the p.Arg1228Ter variant is classified as pathogenic for glycogen storage disease type III. This variant was observed by ICSL as part of a predisposition screen in an ostensibly healthy population. -
