The p.Ala156Gly variant in BAG3 has been identified by our laboratory in 1 infan t with DCM. It has also been identified in 3/9186 African chromosomes by the Exo me Aggregation Consortium (ExAC, http://exac.broadinstitute.org; dbSNP rs5720381 96). Computational prediction tools and conservation analysis suggest that the v ariant may not impact the protein, though this information is not predictive eno ugh to rule out pathogenicity. In summary, the clinical significance of the p.Al a156Gly variant is uncertain. -
Variant summary: BAG3 c.467C>G (p.Ala156Gly) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 2.4e-05 in 1610560 control chromosomes, predominantly at a frequency of 0.00035 within the African or African-American subpopulation in the gnomAD database. The observed variant frequency within African or African-American control individuals in the gnomAD database is approximately 9 fold of the estimated maximal expected allele frequency for a pathogenic variant in BAG3 causing Dilated Cardiomyopathy phenotype (3.9e-05). c.467C>G has been observed in at least an individual affected with Dilated Cardiomyopathy (Burstein_2021). This report, however, does not provide unequivocal conclusions about association of the variant with Dilated Cardiomyopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 32746448). ClinVar contains an entry for this variant (Variation ID: 179298). Based on the evidence outlined above, the variant was classified as likely benign. -

This variant was observed in the ICSL laboratory as part of a predisposition screen in an ostensibly healthy population. It had not been previously curated by ICSL or reported in the Human Gene Mutation Database (HGMD: prior to June 1st, 2018), and was therefore a candidate for classification through an automated scoring system. Utilizing variant allele frequency, disease prevalence and penetrance estimates, and inheritance mode, an automated score was calculated to assess if this variant is too frequent to cause the disease. Based on the score, this variant could not be ruled out of causing disease and therefore its association with disease required further investigation. A literature search was performed for the gene, cDNA change, and amino acid change (if applicable). No publications were found based on this search. This variant was therefore classified as a variant of unknown significance for this disease. -

The p.A156G variant (also known as c.467C>G), located in coding exon 2 of the BAG3 gene, results from a C to G substitution at nucleotide position 467. The alanine at codon 156 is replaced by glycine, an amino acid with similar properties. This variant co-occurred with a variant in another cardiac-related gene in a pediatric dilated cardiomyopathy case (Burstein DS et al. Pediatr Res. 2021 May;89(6):1470-1476f). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
