Variant summary
Our verdict is Benign. The variant received -7 ACMG points: 0P and 7B. BP4_ModerateBS1_SupportingBS2
The NM_004281.4(BAG3):c.508C>T(p.Arg170Trp) variant causes a missense, splice region change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000415 in 1,612,560 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 2/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R170Q) has been classified as Uncertain significance.

Myofibrillar myopathy 6;C3151293:Dilated cardiomyopathy 1HH Uncertain:2
This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 170 of the BAG3 protein (p.Arg170Trp). This variant is present in population databases (rs200479768, gnomAD 0.01%). This variant has not been reported in the literature in individuals affected with BAG3-related conditions. ClinVar contains an entry for this variant (Variation ID: 191607). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

BAG3-related disorder Uncertain:1
The BAG3 c.508C>T variant is predicted to result in the amino acid substitution p.Arg170Trp. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.0085% of alleles in individuals of Latino descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
Cardiovascular phenotype Uncertain:1
The p.R170W variant (also known as c.508C>T) is located in coding exon 3 of the BAG3 gene. The arginine at codon 170 is replaced by tryptophan, an amino acid with dissimilar properties. This change occurs in the first base pair of coding exon 3. This variant co-occurred with a NEXN variant in an individual reported to have hypertrophic cardiomyopathy, and also co-occurred with multiple variants in cardiac-related genes, including a nonsense alteration and large deletion of DSG2, in an individual with arrhythmogenic cardiomyopathy and noncompaction (van Lint FHM et al. Neth Heart J. 2019 Jun;27(6):304-309; Brodehl A et al. Int J Mol Sci. 2021 Apr;22(7)). This alteration has been reported as a secondary cardiac variant in an exome cohort (Ng D et al. Circ Cardiovasc Genet, 2013 Aug;6:337-46). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
