Variant summary
Our verdict is Uncertain significance. The variant received 4 ACMG points: 4P and 0B. PP3_Strong
The NM_032578.4(MYPN):c.1345G>T(p.Gly449Cys) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000434 in 1,613,938 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 13/22 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. G449V) has been classified as Uncertain significance.

Submissions by phenotype
Dilated cardiomyopathy 1KK Uncertain:1
This sequence change replaces glycine with cysteine at codon 449 of the MYPN protein (p.Gly449Cys). The glycine residue is highly conserved and there is a large physicochemical difference between glycine and cysteine. Algorithms developed to predict the effect of missense changes on protein structure and function do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Probably Damaging"; Align-GVGD: "Class C0"). This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a MYPN-related disease. In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance.
Cardiovascular phenotype Uncertain:1
The p.G449C variant (also known as c.1345G>T), located in coding exon 6 of the MYPN gene, results from a G to T substitution at nucleotide position 1345. The glycine at codon 449 is replaced by cysteine, an amino acid with highly dissimilar properties. This amino acid position is conserved. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
