Variant summary
Our verdict is Likely benign. The variant received -1 ACMG points: 0P and 1B. BP4
The NM_032578.4(MYPN):c.2164C>T(p.Arg722Trp) variant causes a missense change. The variant allele was found at a frequency of 0.0000112 in 1,614,018 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R722Q) has been classified as Likely benign.

not provided Uncertain:2
Has not been previously published as pathogenic or benign to our knowledge; In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Reported in ClinVar as a variant of uncertain significance (ClinVar Variant ID# 575119; Landrum et al., 2016); This variant is associated with the following publications: (PMID: 27535533) -
MYPN: PM2 -
Dilated cardiomyopathy 1KK Uncertain:1
This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 722 of the MYPN protein (p.Arg722Trp). This variant is present in population databases (rs749716608, gnomAD 0.005%). This variant has not been reported in the literature in individuals affected with MYPN-related conditions. ClinVar contains an entry for this variant (Variation ID: 575119). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C65"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Cardiovascular phenotype Uncertain:1
The c.2164C>T (p.R722W) alteration is located in exon 11 (coding exon 10) of the MYPN gene. This alteration results from a C to T substitution at nucleotide position 2164, causing the arginine (R) at amino acid position 722 to be replaced by a tryptophan (W). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
