This protein forms membrane pores that allow the release of granzymes and subsequent cytolysis of target cells. Whether pore formation occurs in the plasma membrane of target cells or in an endosomal membrane inside target cells is subject to debate. Mutations in this gene are associated with a variety of human disease including diabetes, multiple sclerosis, lymphomas, autoimmune lymphoproliferative syndrome (ALPS), aplastic anemia, and familial hemophagocytic lymphohistiocytosis type 2 (FHL2), a rare and lethal autosomal recessive disorder of early childhood. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.8e-05 in 251458 control chromosomes. c.916G>A has been reported in the literature in individuals affected with Familial Hemophagocytic Lymphohistiocytosis (Gao_2016, Nagafuji_2007, Wang_2012). These data indicate that the variant may be associated with disease. A different variant affecting the same codon has been classified as pathogenic by our lab (c.916G>T, p.Gly306Cys), supporting the critical relevance of codon 306 to PRF1 protein function. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 26739415, 17606450, 22249210). ClinVar contains an entry for this variant (Variation ID: 468311). Based on the evidence outlined above, the variant was classified as likely pathogenic. -
