Variant summary: PRF1 c.916G>A (p.Gly306Ser) results in a non-conservative amino acid change located in the Membrane attack complex component/perforin (MACPF) domain (IPR020864) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2.8e-05 in 251458 control chromosomes. c.916G>A has been reported in the literature in individuals affected with Familial Hemophagocytic Lymphohistiocytosis (Gao_2016, Nagafuji_2007, Wang_2012). These data indicate that the variant may be associated with disease. A different variant affecting the same codon has been classified as pathogenic by our lab (c.916G>T, p.Gly306Cys), supporting the critical relevance of codon 306 to PRF1 protein function. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 26739415, 17606450, 22249210). ClinVar contains an entry for this variant (Variation ID: 468311). Based on the evidence outlined above, the variant was classified as likely pathogenic. -

This sequence change replaces glycine with serine at codon 306 of the PRF1 protein (p.Gly306Ser). The glycine residue is highly conserved and there is a small physicochemical difference between glycine and serine. This variant is present in population databases (rs763002067, ExAC 0.006%). This missense change has been observed in individual(s) with hemophagocytic lymphohistiocytosis (PMID: 17606450, 22249210, 25233452, 26739415). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
