This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

In silico analysis, which includes protein predictors and evolutionary conservation, supports that this variant does not alter protein structure/function; Observed in individuals with breast and other cancer (Tung 2015, Zhang 2015, Chan 2018, Xie 2018); This variant is associated with the following publications: (PMID: 26580448, 27062340, 25186627, 28580595, 30093976, 25741868) -
The ATM p.Gln95Lys variant was not identified in the literature nor was it identified in the COGR, Cosmic, MutDB, and LOVD 3.0. The variant was identified in dbSNP (ID: rs587781545) as “With Pathogenic, Uncertain significance allele”, in ClinVar (classified as uncertain significance by GeneDx, Invitae, Color Genomics Inc, and Ambry Genetics), Clinvitae (3x), and in control databases in 34 of 276752 chromosomes (1 homozygous) at a frequency of 0.0001 (Genome Aggregation Database Feb 27, 2017). It was observed in the East Asian populations in 34 (1 homozygous) of 18838 chromosomes (freq: 0.002), but not in the African, Other, Latino, European Non-Finnish, Ashkenazi Jewish, European Finnish, and South Asian populations. The p.Gln95 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact of the variant Lys to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as uncertain significance. -

Variant summary: ATM c.283C>A (p.Gln95Lys) results in a conservative amino acid change located in the Telomere-length maintenance and DNA damage repair domain (IPR021668) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 0.00012 in 250948 control chromosomes, predominantly at a frequency of 0.0017 within the East Asian subpopulation in the gnomAD database, including 1 homozygotes. The observed variant frequency within East Asian control individuals in the gnomAD database is approximately 2 fold of the estimated maximal expected allele frequency for a pathogenic variant in ATM causing Breast Cancer phenotype (0.001). c.283C>A has been reported in the literature as a VUS in settings of multigene panel testing in individuals of East Asian ancestry affected with Breast Cancer (e.g. Chan_2018, Tung_2015, Wang_2019, Xie_2018, Kwong_2020). These reports do not provide unequivocal conclusions about association of the variant with Breast Cancer. These report(s) do not provide unequivocal conclusions about association of the variant with Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publications have been ascertained in the context of this evaluation (PMID: 25186627, 26580448, 30093976, 28580595, 30982232, 31360874, 32068069). ClinVar contains an entry for this variant (Variation ID: 181957). Based on the evidence outlined above, the variant was classified as likely benign. -
