Variant summary: ATM c.489G>T (p.Gln163His) results in a non-conservative amino acid change located in the Telomere-length maintenance and DNA damage repair domain (IPR021668) of the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251304 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.489G>T in individuals affected with Breast Cancer and no experimental evidence demonstrating its impact on protein function have been reported. Co-occurrence with another likely pathogenic variant has been reported (PTEN c.395G>A, p.Gly132Asp) in our internal database, providing supporting evidence for a benign role. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance.

This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 163 of the ATM protein (p.Gln163His).
Hereditary cancer-predisposing syndrome Uncertain:1
The p.Q163H variant (also known as c.489G>T), located in coding exon 4 of the ATM gene, results from a G to T substitution at nucleotide position 489. The glutamine at codon 163 is replaced by histidine, an amino acid with highly similar properties. This amino acid position is conserved. In addition, this alteration is predicted to be tolerated by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear.
