This missense change has been observed in individual(s) with cerebellar ataxia (PMID: 31429931). This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 256 of the ATM protein (p.Asp256Val). This variant is not present in population databases (gnomAD no frequency). ClinVar contains an entry for this variant (Variation ID: 482738). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
Hereditary cancer-predisposing syndrome Uncertain:1
The p.D256V variant (also known as c.767A>T), located in coding exon 6 of the ATM gene, results from an A to T substitution at nucleotide position 767. The aspartic acid at codon 256 is replaced by valine, an amino acid with highly dissimilar properties. This alteration was identified in an individual diagnosed with early onset cerebellar ataxia (Shakya S et al. Clin Genet, 2019 Dec;96:566-574). This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
