Variant summary: ATM c.1689G>C (p.Met563Ile) results in a conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 251282 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.1689G>C has been reported in the literature as a VUS in at-least one Japanese individuals affected with breast cancer (example, Momozawa_2018). These report(s) do not provide unequivocal conclusions about association of the variant with Ataxia-Telangiectasia/breast cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Although two other nucleotide changes, namely c.1689G>A and c.1689G>T both of which result in the same protein effect, namely p.Met563Ile, have been submitted with a clinical significance assessment of VUS/likely benign and VUS respectively to ClinVar after 2014. Based on the evidence outlined above, the variant was classified as uncertain significance.

This missense change has been observed in individual(s) with breast cancer (PMID: 30287823). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt ATM protein function. ClinVar contains an entry for this variant (Variation ID: 1172872). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces methionine, which is neutral and non-polar, with isoleucine, which is neutral and non-polar, at codon 563 of the ATM protein (p.Met563Ile).
