ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. The ATM c.4703A>G (p.His1568Arg) variant has been reported in the published literature in individuals with personal and/or family history of breast and/or ovarian cancer (PMIDs: 31159747 (2019), 32957588 (2020), 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)) and chronic lymphocytic leukemia (PMID: 28652578 (2017)). It has also been found in reportedly healthy individuals (PMIDs: 30287823 (2018), 31159747 (2019), 33471991 (2021), see also LOVD (http://databases.lovd.nl/shared)). The frequency of this variant in the general population, 0.000088 (10/113434 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. The variant was identified in dbSNP (rs368830730) as “with uncertain significance allele”, ClinVar (classified as uncertain significance by GeneDx, Invitae, Ambry Genetics and GeneKor; and as likely benign by Color) and LOVD 3.0 (observed 1x). The variant was identified in control databases in 10 of 245,814 chromosomes at a frequency of 0.00004 (Genome Aggregation Database Feb 27, 2017). The variant was observed in the European population in 10 of 111,368 chromosomes (freq: 0.00009), but not in the African, Other, Latino, Ashkenazi Jewish, East Asian, Finnish, or South Asian populations. The p.His1568 residue is not conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest a high likelihood of impact to the protein; however, this information is not predictive enough to rule out pathogenicity.
