Genes affected
ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Mutations in this gene are associated with ataxia telangiectasia, an autosomal recessive disorder. [provided by RefSeq, Aug 2010]
C11orf65 (HGNC:28519): (chromosome 11 open reading frame 65) Predicted to be involved in negative regulation of mitochondrial fission and negative regulation of protein targeting to mitochondrion. Predicted to be located in cytosol and mitochondrial outer membrane. [provided by Alliance of Genome Resources, Apr 2022]

Significance:Pathogenic
Review Status:criteria provided, single submitter
Collection Method:clinical testing

This variant changes 1 nucleotide in exon 43 of the ATM gene, creating a premature translation stop signal. This variant is expected to result in an absent or non-functional protein product. To our knowledge, this variant has not been reported in individuals affected with hereditary cancer in the literature. This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of ATM function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Pathogenic.

The p.E2097* pathogenic mutation (also known as c.6289G>T), located in coding exon 42 of the ATM gene, results from a G to T substitution at nucleotide position 6289. This changes the amino acid from a glutamic acid to a stop codon within coding exon 42. This alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation.

PVS1, PM2_Supporting, PM5_Supporting c.6289G>T, located in exon 43 of the ATM gene, is a nonsense variant expected to result in loss of function by premature protein truncation and nonsense-mediated mRNA decay (PVS1). It is predicted to convey a premature termination upstream codon 3047 (PM5_Supporting). It is not present in the population database gnomAD v2.1.1, non-cancer dataset (PM2_Supporting). The SpliceAI algorithm predicts no significant impact on splicing. To our knowledge, neither relevant clinical data nor well-established functional studies have been reported for this variant. This variant has been reported in the ClinVar database (4x pathogenic, 1x likely pathogenic), and has not been reported in the LOVD database. Based on currently available information, the variant c.6289G>T should be considered a pathogenic variant according to ACMG Classification Rules Specified for ATM v1.1.

This sequence change creates a premature translational stop signal (p.Glu2097*) in the ATM gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in ATM are known to be pathogenic (PMID: 23807571, 25614872). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with ATM-related conditions. ClinVar contains an entry for this variant (Variation ID: 478929). For these reasons, this variant has been classified as Pathogenic.
