This variant is present in population databases (rs768155385, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with ATM-related conditions. ClinVar contains an entry for this variant (Variation ID: 418968). Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated. The ATM p.Tyr2167Cys variant was not identified in the literature nor was it identified in the LOVD 3.0 database. The variant was identified in dbSNP (ID: rs768155385) as “With Uncertain significance allele” and ClinVar (classified as uncertain significance by GeneDx, Invitae, Ambry Genetics and Color). The variant was identified in control databases in 1 of 246222 chromosomes at a frequency of 0.000004 (Genome Aggregation Database Feb 27, 2017), observed in the European Non-Finnish population in 1 of 111678 chromosomes (freq: 0.000009); it was not observed in the African, Other, Latino, Ashkenazi Jewish, East Asian, European Finnish, or South Asian populations. The p.Tyr2167 residue is conserved in mammals and computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) provide inconsistent predictions regarding the impact of the Cys variant to the protein; this information is not very predictive of pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer) do not predict a difference in splicing. The p.Y2167C variant (also known as c.6500A>G), located in coding exon 44 of the ATM gene, results from an A to G substitution at nucleotide position 6500. The tyrosine at codon 2167 is replaced by cysteine, an amino acid with highly dissimilar properties. This variant was observed in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879) and in 2/2870 individuals as part of a large Canadian cohort study (Bhai P et al. Front Genet, 2021 Jul;12:698595). This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -

Variant summary: ATM c.6500A>G (p.Tyr2167Cys) results in a non-conservative amino acid change located in the PIK-related kinase, FAT domain (IPR003151) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251448 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.6500A>G has been reported in the literature in at least one individual affected with Breast Cancer without evidence for causality (e.g. Rezoug_2024). This report does not provide unequivocal conclusions about association of the variant with Breast Cancer. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 39226054). ClinVar contains an entry for this variant (Variation ID: 418968). This variant was reported in an individual undergoing hereditary breast and ovarian cancer testing who had a diagnosis of leiomyosarcoma (Lerner-Ellis et al. 2020. PubMed ID: 32885271), as well as individuals with breast cancer and colorectal cancer (Bhai et al. 2021. PubMed ID: 34326862). This variant is reported in 0.00088% of alleles in individuals of European (Non-Finnish) descent in gnomAD and is interpreted as a variant of uncertain significance in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/418968/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence. -
