Genes affected
ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Mutations in this gene are associated with ataxia telangiectasia, an autosomal recessive disorder. [provided by RefSeq, Aug 2010]
C11orf65 (HGNC:28519): (chromosome 11 open reading frame 65) Predicted to be involved in negative regulation of mitochondrial fission and negative regulation of protein targeting to mitochondrion. Predicted to be located in cytosol and mitochondrial outer membrane. [provided by Alliance of Genome Resources, Apr 2022]

Uncertain significance, criteria provided, single submitterclinical testingGeneDxJun 07, 2023Observed with a truncating ATM variant in three individuals from the same family with cerebellar ataxia and/or peripheral neuropathy and extrapyramidal features, but it is not known whether the variants occurred on the same (in cis) allele or on opposite (in trans) alleles (Schon et al., 2019); Published functional studies suggest this variant retains some residual kinase activity: showed reduced ATM expression, absence of autophosphorylation, and reduced levels of phosphorylation of downstream targets (Taylor et al., 2015); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Not observed at significant frequency in large population cohorts (gnomAD); This variant is associated with the following publications: (PMID: 22952040, 23532176, 30549301, 25040471) -

Hereditary cancer-predisposing syndrome Uncertain:2
Uncertain significance, criteria provided, single submitterclinical testingAmbry GeneticsFeb 21, 2024The p.D2395V variant (also known as c.7184A>T), located in coding exon 48 of the ATM gene, results from an A to T substitution at nucleotide position 7184. The aspartic acid at codon 2395 is replaced by valine, an amino acid with highly dissimilar properties. This variant has been identified in multiple individuals with breast cancer (Decker B et al. J Med Genet, 2017 Nov;54:732-741; Bhai P et al. Front Genet, 2021 Jul;12:698595). Additionally, This variant has been identified in conjunction with another ATM variant in three related individuals with features consistent with variant ataxia-telangiectasia; however, the phase of the two variants was not specified (Schon K et al. Ann Neurol, 2019 Feb;85:170-180). . Immunoblots conducted on patient cells with this alteration show reduced but not absent protein with reduced kinase activity (Taylor AM et al. Clin. Genet., 2015 Mar;87:199-208; Ouillette P et al. Genes Chromosomes Cancer, 2012 Dec;51:1125-32). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this alteration remains unclear. -
Uncertain significance, criteria provided, single submitterclinical testingColor Diagnostics, LLC DBA Color HealthJul 25, 2023This missense variant replaces aspartic acid with valine at codon 2395 of the ATM protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). Experimental functional studies with this variant have reported reduced ATM kinase activity and ATM protein levels in patient cells, indicating the variant may be hypomorphic (PMID: 25040471). This variant has been reported biallelic (with ATM p.Glu2164*) in three individuals from one family affected with mild or variant ataxia-telangiectasia (A-T) in the literature, but the phase of these alleles is unknown (PMID: 30549301). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Ataxia-telangiectasia syndrome Uncertain:1
Uncertain significance, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpAug 16, 2023This missense change has been observed in individual(s) with clinical features of ATM-related conditions (PMID: 25040471). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Experimental studies have shown that this missense change affects ATM function (PMID: 22952040, 25040471). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. ClinVar contains an entry for this variant (Variation ID: 485217). This variant is not present in population databases (gnomAD no frequency). This sequence change replaces aspartic acid, which is acidic and polar, with valine, which is neutral and non-polar, at codon 2395 of the ATM protein (p.Asp2395Val). -
