Variant summary
Our verdict is Likely pathogenic. The variant received 6 ACMG points: 6P and 0B. PM1PM2PP3_Moderate
The NM_000051.4(ATM):c.8187A>T(p.Gln2729His) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000205 in 1,461,762 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another nucleotide change resulting in the same amino acid substitution has been previously reported as Uncertain significance in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. Q2729E) has been classified as Uncertain significance.

Identified in patients with breast cancer and prostate cancer in published literature (PMID: 29678143, 24556621, 35127508); Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; This variant is associated with the following publications: (PMID: 23532176, 15279808, 30287823, 29678143, 35127508, 36243179, 24556621) -
Hereditary cancer-predisposing syndrome Uncertain:2
This missense variant replaces glutamine with histidine at codon 2729 of the ATM protein. Computational prediction suggests that this variant may have deleterious impact on protein structure and function (internally defined REVEL score threshold >= 0.7, PMID: 27666373). To our knowledge, functional studies have not been performed for this variant. This variant has been reported in an individual affected with breast cancer (PMID: 29678143). A different variant that results in the same amino acid change, c.8187A>C (p.Gln2729His), has been reported in individuals with breast cancer and prostate cancer (PMID: 29335925, 24556621, 35127508, 33471991). The variant c.8187A>C (p.Gln2729His) has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
The p.Q2729H variant (also known as c.8187A>T), located in coding exon 55 of the ATM gene, results from an A to T substitution at nucleotide position 8187. The glutamine at codon 2729 is replaced by histidine, an amino acid with highly similar properties. This alteration has been identified in an individual diagnosed with breast cancer (Podralska M et al. BMC Cancer, 2018 Apr;18:452). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
Ataxia-telangiectasia syndrome Uncertain:1
This sequence change replaces glutamine, which is neutral and polar, with histidine, which is basic and polar, at codon 2729 of the ATM protein (p.Gln2729His). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer (PMID: 29335925, 29678143, 30287823, 35534704). ClinVar contains an entry for this variant (Variation ID: 181986). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt ATM protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
