ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. The glutamine at codon 2989 is replaced by glutamic acid, an amino acid with highly similar properties. This alteration has been reported in a Turkish family with early-onset breast cancer, in which affected individuals also harbored a truncating BRCA2 mutation (Celik E et al. Clin Case Rep. 2018 Sep;6:1751-1755). In another study, this alteration was also observed in 1/3251 individuals who met eligibility criteria for hereditary breast and ovarian cancer syndrome (Lerner-Ellis J et al. J Cancer Res Clin Oncol, 2021 Mar;147:871-879). This alteration was also seen in 1/732 breast cancer patients, 0/189 colorectal cancer patients and 0/490 cancer-free elderly controls in a Turkish population (Akcay IM et al. Int J Cancer, 2021 Jan;148:285-295). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Four of four in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 251390 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.8965C>G has been reported in the literature in individuals affected with Breast Cancer (Celik_2018, Bakos_2021), Chronic Lymphocytic Leukemia (Tiao_2017), and Lynch Syndrome (Yurgelun_2015). These report(s) do not provide unequivocal conclusions about association of the variant with Breast Cancer. Co-occurrences with a pathogenic variant were reported in one Breast Cancer patient (BRCA2 c.7655_7658del, p.Ile2552fs, Celik_2018) as well as a Lynch Syndrome patient (MSH2 del exon 8, Yurgelun_2015), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Six clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as VUS. -

The variant was identified in the following databases: dbSNP (ID: rs147695170) “With Uncertain significance allele”, ClinVar (classified uncertain significance by Ambry Genetics, GeneDx and Invitae), Clinvitae (3x), and in control databases in 3 of 246156 chromosomes at a frequency of 0.00001 increasing the likelihood that this may be a low frequency variant in certain populations of origin (Genome Aggregation Consortium Feb 27, 2017), It was observed in the European Non-Finnish population in 3 of 111618 chromosomes (freq: 0.00001) and was not seen in African, Other, Latino, Ashkenazi Jewish, East Asian, European Finnish and South Asian populations. The p.Gln2989 residue is conserved in mammals but not in more distantly related organisms however computational analyses (PolyPhen-2, SIFT, AlignGVGD, BLOSUM, MutationTaster) do not suggest the Glu residue has a high likelihood of impact to the protein; this information is not predictive enough to rule out pathogenicity. The variant occurs outside of the splicing consensus sequence and in silico or computational prediction software programs (SpliceSiteFinder, MaxEntScan, NNSPLICE, GeneSplicer, HumanSpliceFinder) do not predict a difference in splicing. In summary, based on the above information the clinical significance of this variant cannot be determined with certainty at this time. This variant is classified as a variant of uncertain significance. -
