Variant summary
Our verdict is Uncertain significance. Variant got 1 ACMG points: 2P and 1B. PM2BP4
The NM_000482.4(APOA4):c.998C>T(p.Ala333Val) variant causes a missense change. The variant allele was found at a frequency of 0.000065 in 1,614,156 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 12/19 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. A333A) has been classified as Likely benign.

not specified Uncertain:1
The c.998C>T (p.A333V) alteration is located in exon 3 (coding exon 3) of the APOA4 gene. This alteration results from a C to T substitution at nucleotide position 998, causing the alanine (A) at amino acid position 333 to be replaced by a valine (V). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
not provided Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). This variant has not been reported in the literature in individuals affected with APOA4-related conditions. This variant is present in population databases (rs113263292, ExAC 0.01%). This sequence change replaces alanine with valine at codon 333 of the APOA4 protein (p.Ala333Val). The alanine residue is moderately conserved and there is a small physicochemical difference between alanine and valine. -
