Genes affected
HMBS (HGNC:4982): (hydroxymethylbilane synthase) This gene encodes a member of the hydroxymethylbilane synthase superfamily. The encoded protein is the third enzyme of the heme biosynthetic pathway and catalyzes the head to tail condensation of four porphobilinogen molecules into the linear hydroxymethylbilane. Mutations in this gene are associated with the autosomal dominant disease acute intermittent porphyria. Alternatively spliced transcript variants encoding different isoforms have been described. ACMG classification

Classification made for transcript

Verdict is Pathogenic. Variant got 18 ACMG points.

PVS1
Loss of function variant, product undergoes nonsense mediated mRNA decay. LoF is a known mechanism of disease.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 11-119092958-G-A is Pathogenic according to our data. Variant chr11-119092958-G-A is described in ClinVar as [Pathogenic]. Clinvar id is 1483.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

Pathogenic, criteria provided, single submitterclinical testingGeneDxJul 02, 2024Previously reported in a parent and child with acute intermittent porphyria (AIP) (PMID: 7962538); Published functional studies demonstrate that the variant results in significantly reduced enzyme activity (PMID: 7962538); Not observed at significant frequency in large population cohorts (gnomAD); Nonsense variant predicted to result in protein truncation or nonsense mediated decay in a gene for which loss of function is a known mechanism of disease; This variant is associated with the following publications: (PMID: 25525159, 7962538, 34308104, 35753512, 12566739) -
Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpDec 22, 2023This sequence change creates a premature translational stop signal (p.Trp283*) in the HMBS gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in HMBS are known to be pathogenic (PMID: 7757070, 7962538). This variant is not present in population databases (gnomAD no frequency). This premature translational stop signal has been observed in individuals with autosomal dominant acute intermittent porphyria. It is commonly reported in individuals of Swiss ancestry. (PMID: 7962538, 12566739). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 1483). For these reasons, this variant has been classified as Pathogenic. -
