The c.2222+15C>A variant in ABCC8 has been previously reported in at least 3 individuals with hyperinsulinemic hypoglycemia (PMID: 28442472, 34336745), and has been seen in 0.005% (56/1179598) of European (non-Finnish) chromosomes by the Genome Aggregation Database (gnomAD, http://gnomad.broadinstitute.org; dbSNP: rs377174421). Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. This variant has also been reported in ClinVar (VCV000558288.6) and has been interpreted as likely pathogenic by Labcorp Genetics and as a variant of uncertain significance by Fulgent Genetics and Counsyl. This variant is located in the 3’ splice region. Computational tools do not suggest an impact to splicing. However, this information is not predictive enough to rule out pathogenicity. In summary, the clinical significance of the c.2222+15C>A variant is uncertain. ACMG/AMP Criteria applied: PM3, PM2_supporting (Richards 2015). -

This sequence change falls in intron 16 of the ABCC8 gene. It does not directly change the encoded amino acid sequence of the ABCC8 protein. This variant is present in population databases (rs377174421, gnomAD 0.0009%). This variant has been observed in individual(s) with autosomal recessive familial hyperinsulinism (PMID: 28442472). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 558288). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant is not likely to affect RNA splicing. In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -
