Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes the canonical 5' splicing donor site. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 4e-06 in 251248 control chromosomes. c.1630+1G>A has been reported in the literature as a compound heterozygous genotype in at-least one individual affected with the diffuse form of Congenital Hyperinsulinism (example, Henwood_2005, cited in Snider_2013). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. Based on data from gnomAD, the A allele has an overall frequency of <0.01% (1/251248) total alleles studied. Another alteration impacting the same donor site (c.1630+1G>T) has been described in conjunction with other ABCC8 mutations in individuals with familial hyperinsulinism (Nestorowicz, 1998; Salisbury, 2015; Sandal, 2009). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site. Although this variant has been seen in the general population in a heterozygous state, its frequency is low enough to be consistent with a recessive carrier frequency. Of the 3 affected individuals, 1 was a compound heterozygote that carried a reported pathogenic variant with unknown phase, which increases the likelihood that the c.1630+1G>A variant is pathogenic (PMID: 23275527). This variant is located in the 3' splice region. SpliceAI predictions indicate use of an out-of-frame cryptic splice site 70 bases from the intron-exon boundary, providing evidence that this variant may cause a frameshift and lead to a premature termination codon downstream. This alteration is then predicted to lead to a truncated or absent protein. However, this information is not predictive enough to determine pathogenicity. Loss of function of the ABCC8 gene is an established disease mechanism in autosomal recessive hyperinsulinemic hypoglycemia. In summary, this variant meets criteria to be classified as pathogenic for autosomal recessive hyperinsulinemic hypoglycemia.
