Variant summary
Our verdict is Uncertain significance. The variant received 0 ACMG points: 1P and 1B. PP2BP4
The NM_000218.3(KCNQ1):c.197C>A(p.Ser66Tyr) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000121 in 1,400,894 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 13/23 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. S66F) has been classified as Uncertain significance.

Has not been previously published as pathogenic or benign to our knowledge; In silico analysis supports that this missense variant does not alter protein structure/function -
Long QT syndrome Uncertain:1
This sequence change replaces serine, which is neutral and polar, with tyrosine, which is neutral and polar, at codon 66 of the KCNQ1 protein (p.Ser66Tyr). This variant is present in population databases (no rsID available, gnomAD 0.03%). This missense change has been observed in individual(s) with clinical features of long QT syndrome (internal data). ClinVar contains an entry for this variant (Variation ID: 933445). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed for this missense variant. However, the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on KCNQ1 protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Cardiovascular phenotype Uncertain:1
The p.S66Y variant (also known as c.197C>A), located in coding exon 1 of the KCNQ1 gene, results from a C to A substitution at nucleotide position 197. The serine at codon 66 is replaced by tyrosine, an amino acid with dissimilar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
