Myosin-binding protein C is a myosin-associated protein found in the cross-bridge-bearing zone (C region) of A bands in striated muscle. MYBPC3 is expressed exclusively in heart muscle and is a key regulator of cardiac contraction. Mutations in this gene are a frequent cause of familial hypertrophic cardiomyopathy. The c.3330+2 T>C mutation in the MYBPC3 gene has been reported previously in association with HCM (Waldmuller S et al., 2008). In addition, a different nucleotide substitution at this position, c.3330+2 T>G, also results in a splice site mutation and has been reported multiple times in association with HCM (Xin B et al., 2007; Morita H et al., 2008). The c.3330+2 T>C mutation destroys the consensus splice donor site of intron 30, and is expected to cause abnormal gene splicing. This mutation is expected to lead to either an abnormal message that is subject to nonsense-mediated mRNA decay, or to an abnormal protein product if the message is used for protein translation. Other splice site mutations in the MYBPC3 gene have been reported in association with HCM. In summary, c.3330+2 T>C in the MYBPC3 gene is interpreted as a disease-causing mutation. The variant is found in HCM panel(s). -

It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in MYBPC3 are known to be pathogenic (PMID: 19574547). This variant is not present in population databases (gnomAD no frequency). Disruption of this splice site has been observed in individuals with hypertrophic cardiomyopathy (PMID: 17937428, 18258667, 18467358). ClinVar contains an entry for this variant (Variation ID: 181007). Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. For these reasons, this variant has been classified as Pathogenic. -
