Variant summary: MYBPC3 c.3330+2T>A is located in a canonical splice-site and is predicted to affect mRNA splicing resulting in a significantly altered protein due to either exon skipping, shortening, or inclusion of intronic material. Several computational tools predict a significant impact on normal splicing: Four predict the variant abolishes a 5' splicing donor site. However, these predictions have yet to be confirmed by functional studies. The variant was absent in 228414 control chromosomes (gnomAD). To our knowledge, no occurrence of c.3330+2T>A in individuals affected with Cardiomyopathy and no experimental evidence demonstrating its impact on protein function have been reported. No clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014. Other variants at the same position (i.e. c.3330+2T>C, c.3330+2T>G) have been classified as pathogenic/likely pathogenic via internal testing and in ClinVar. Based on the evidence outlined above, the variant was classified as likely pathogenic. -

The c.3330+2T>A intronic pathogenic mutation results from a T to A substitution two nucleotides after coding exon 30 in the MYBPC3 gene. This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site and may result in the creation or strengthening of a novel splice donor site. Another alteration impacting the same donor site (c.3330+2T>G) has been detected in multiple individuals with hypertrophic cardiomyopathy, and RNA studies have indicated that the variant results in exon 30 skipping (Theis JL et al. Circ Heart Fail. 2009;2:325-33; Miller EM et al. J Genet Couns. 2013;22:258-67; Xin B et al. Am. J. Med. Genet. A. 2007;143A(22):2662-7). Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. As such, this alteration is classified as a disease-causing mutation. -
