Myosin-binding protein C is a myosin-associated protein found in the cross-bridge-bearing zone (C region) of A bands in striated muscle. MYBPC3 is expressed exclusively in heart muscle and is a key regulator of cardiac contraction. Mutations in this gene are a frequent cause of familial hypertrophic cardiomyopathy. The arginine at codon 335 is replaced by cysteine, an amino acid with highly dissimilar properties. This alteration was identified in a case of unexplained sudden cardiac death in a one year old male (Bagnall RD et al. N. Engl. J. Med., 2016 Jun;374:2441-52). Based on data from ExAC, the T allele has an overall frequency of <0.01% (2/94634). This amino acid position is highly conserved through mammals but not in all available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this variant remains unclear. - |
