Variant summary
Our verdict is Likely benign. The variant received -3 ACMG points: 3P and 6B. PM1PP2BP4BP6BS2
The NM_001370259.2(MEN1):c.566A>G(p.Asn189Ser) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.0000136 in 1,614,144 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. N189K) has been classified as Likely benign.

In silico analysis supports that this missense variant does not alter protein structure/function; Observed in an individual with acute lymphoblastic leukemia (Zhang et al., 2015); This variant is associated with the following publications: (PMID: 9989505, 35268848, 26580448)
Hereditary cancer-predisposing syndrome Uncertain:1
The p.N189S variant (also known as c.566A>G), located in coding exon 2 of the MEN1 gene, results from an A to G substitution at nucleotide position 566. The asparagine at codon 189 is replaced by serine, an amino acid with highly similar properties. This amino acid position is not well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
