Variant summary
Our verdict is Likely benign. The variant received -2 ACMG points: 0P and 2B. BP4_Moderate
The NM_016938.5(EFEMP2):c.348C>A(p.Asp116Glu) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.000152 in 1,614,080 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. D116N) has been classified as Uncertain significance.

Submissions by phenotype
Cutis laxa, autosomal recessive, type 1B Uncertain:2
This sequence change replaces aspartic acid, which is acidic and polar, with glutamic acid, which is acidic and polar, at codon 116 of the EFEMP2 protein (p.Asp116Glu). This variant is present in population databases (rs376350227, gnomAD 0.04%). This variant has not been reported in the literature in individuals affected with EFEMP2-related conditions. ClinVar contains an entry for this variant (Variation ID: 582876). Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change (SIFT: "Deleterious"; PolyPhen-2: "Benign"; Align-GVGD: "Class C0"). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
This variant was determined to be of uncertain significance according to ACMG Guidelines, 2015 [PMID:25741868].
not provided Uncertain:2
Has not been previously published as pathogenic or benign to our knowledge; In silico analysis supports that this missense variant does not alter protein structure/function
not specified Uncertain:1
Cardiovascular phenotype Uncertain:1
The p.D116E variant (also known as c.348C>A), located in coding exon 3 of the EFEMP2 gene, results from a C to A substitution at nucleotide position 348. The aspartic acid at codon 116 is replaced by glutamic acid, an amino acid with highly similar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
EFEMP2-related disorder Uncertain:1
The EFEMP2 c.348C>A variant is predicted to result in the amino acid substitution p.Asp116Glu. To our knowledge, this variant has not been reported in the literature. This variant is reported in 0.040% of alleles in individuals of Latino descent in gnomAD. At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
