Genes affected
MED17 (HGNC:2375): (mediator complex subunit 17) The activation of gene transcription is a multistep process that is triggered by factors that recognize transcriptional enhancer sites in DNA. These factors work with co-activators to direct transcriptional initiation by the RNA polymerase II apparatus. The protein encoded by this gene is a subunit of the CRSP (cofactor required for SP1 activation) complex, which, along with TFIID, is required for efficient activation by SP1. This protein is also a component of other multisubunit complexes e.g. thyroid hormone receptor-(TR-) associated proteins which interact with TR and facilitate TR function on DNA templates in conjunction with initiation factors and cofactors. ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant located near the start codon (<100nt), not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.976 CDS is truncated, and there are 2 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 11-93784559-G-T is Pathogenic according to our data. Variant chr11-93784559-G-T is described in ClinVar as [Pathogenic]. Clinvar id is 2818918.Status of the report is criteria_provided_single_submitter, 1 stars.

Pathogenic, criteria provided, single submitterclinical testingLabcorp Genetics (formerly Invitae), LabcorpFeb 21, 2023This sequence change creates a premature translational stop signal (p.Glu16*) in the MED17 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in MED17 are known to be pathogenic (PMID: 20950787, 26004231, 30345598). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with MED17-related conditions. For these reasons, this variant has been classified as Pathogenic. -
