Genes affected
POLE (HGNC:9177): (DNA polymerase epsilon, catalytic subunit) This gene encodes the catalytic subunit of DNA polymerase epsilon. The enzyme is involved in DNA repair and chromosomal DNA replication. Mutations in this gene have been associated with colorectal cancer 12 and facial dysmorphism, immunodeficiency, livedo, and short stature. [provided by RefSeq, Sep 2013]

This sequence change affects a donor splice site in intron 40 of the POLE gene. It is expected to disrupt RNA splicing. Variants that disrupt the donor or acceptor splice site typically lead to a loss of protein function (PMID: 16199547), and loss-of-function variants in POLE are known to be pathogenic (PMID: 23230001, 25948378, 30503519). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 517228). Studies have shown that disruption of this splice site is associated with inconclusive levels of altered splicing (internal data). In summary, the currently available evidence indicates that the variant is pathogenic, but additional data are needed to prove that conclusively. Therefore, this variant has been classified as Likely Pathogenic. -

The c.5552+1G>A variant in POLE has not been previously reported in individuals with colorectal cancer and was absent from large population studies. This varian t occurs in the invariant region (+/- 1,2) of the splice consensus sequence and is predicted to cause altered splicing leading to an abnormal or absent protein. Although this variant is expected to severely impact the protein, the spectrum of disease-causing variants is not well defined for POLE (to date, the majority of variants reported in patients with colorectal cancer represent missense chang es). In summary, the clinical significance of the c.5552+1G>A variant is uncerta in. -

Collection Method: clinical testing

The c.5552+1G>A intronic variant results from a G to A substitution one nucleotide after coding exon 40 of the POLE gene. Alterations that disrupt the canonical splice site are expected to cause aberrant splicing, resulting in an abnormal protein or a transcript that is subject to nonsense-mediated mRNA decay. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice donor site resulting in a transcript predicted to lead to a protein with an in-frame deletion of 58 amino acids; however, the exact functional impact of the deleted amino acids is unknown at this time (Ambry internal data). Based on the available evidence, the clinical significance of this variant remains unclear. -
