Variant summary: POLE c.4444+3A>G alters a conserved nucleotide located close to a canonical splice site and therefore could affect mRNA splicing, leading to a significantly altered protein sequence. Several computational tools predict a significant impact on normal splicing: Two predict the variant weakens a 5' donor site. Two predict the variant no significant impact on splicing. At least one publication reports experimental evidence that this variant affects mRNA splicing. The variant was absent in 251402 control chromosomes. c.4444+3A>G has been reported in the literature in multiple individuals affected with Facial Dysmorphism, Immunodeficiency, Livedo, And Short Stature. These data indicate that the variant is very likely to be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function and showed that this variant results in severely decreased expression of full-length protein. The following publication have been ascertained in the context of this evaluation (PMID: 23230001). ClinVar contains an entry for this variant (Variation ID: 41417). Based on the evidence outlined above, the variant was classified as pathogenic. -

This sequence change falls in intron 34 of the POLE gene. It does not directly change the encoded amino acid sequence of the POLE protein. RNA analysis indicates that this variant induces altered splicing and may result in an absent or altered protein product. This variant is present in population databases (rs398122515, gnomAD 0.007%). This variant has been observed in individual(s) with facial dysmorphism, immunodeficiency, livedo, and short stature (FILS syndrome) (PMID: 23230001, 25948378). It has also been observed to segregate with disease in related individuals (PMID: 23230001, 25948378). However, it has not been observed in individuals with colonic adenomatous polyps or colon cancer. ClinVar contains an entry for this variant (Variation ID: 41417). Variants that disrupt the consensus splice site are a relatively common cause of aberrant splicing (PMID: 17576681, 9536098). Studies have shown that this variant results in skipping of exon 34, and produces a non-functional protein and/or introduces a premature termination codon (PMID: 23230001). For these reasons, this variant has been classified as Pathogenic. -

The c.4444+3A>G intronic variant results from an A to G substitution 3 nucleotides after coding exon 34 in the POLE gene. This alteration has been reported as homozygous in individuals with FILS syndrome (Pachlopnik Schmid J et al. J. Exp. Med., 2012 Dec;209:2323-30; Thiffault I et al. BMC Med. Genet., 2015 May;16:31). It has not been reported in an individual with a diagnosis of polymerase proofreading-associated polyposis (PPAP) to date. This nucleotide position is well conserved in available vertebrate species. Using the BDGP and ESEfinder splice site prediction tools, this alteration is not predicted to have any significant effect on this splice donor site; however, direct evidence is unavailable. Based on the available evidence, the contribution of c.4444+3A>G to a clinical diagnosis of PPAP remains unclear. -
