Variant summary
Our verdict is Uncertain significance. The variant received 0 ACMG points: 0P and 0B.
The NM_006231.4(POLE):c.3923G>A(p.Arg1308Gln) variant causes a missense change. The variant allele was found at a frequency of 0.0000186 in 1,613,236 control chromosomes in the GnomAD database, with no homozygous occurrence. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R1308W) has been classified as Uncertain significance.

This sequence change replaces arginine, which is basic and polar, with glutamine, which is neutral and polar, at codon 1308 of the POLE protein (p.Arg1308Gln). This variant is present in population databases (rs759793243, gnomAD 0.007%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 405697). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt POLE protein function with a negative predictive value of 80%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Has not been previously published as pathogenic or benign to our knowledge -
Colorectal cancer, susceptibility to, 12 Uncertain:1
The POLE c.3923G>A p.(Arg1308Gln) missense change has a maximum subpopulation frequency of 0.0064% in gnomAD v2.1.1 (https://gnomad.broadinstitute.org). The in silico tool REVEL predicts a benign effect on protein function, but to our knowledge this prediction has not been confirmed by functional studies. To our knowledge, this variant has not been reported in the literature in individuals with POLE-related disease. In summary, the evidence currently available is insufficient to determine the clinical significance of this variant. It has therefore been classified as of uncertain significance. -
Hereditary cancer-predisposing syndrome Uncertain:1
The c.3923G>A (p.R1308Q) alteration is located in exon 31 (coding exon 31) of the POLE gene. This alteration results from a G to A substitution at nucleotide position 3923, causing the arginine (R) at amino acid position 1308 to be replaced by a glutamine (Q). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear. -
