This sequence change replaces arginine, which is basic and polar, with glycine, which is neutral and non-polar, at codon 712 of the POLE protein (p.Arg712Gly). This variant is present in population databases (rs749194160, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 240426). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt POLE protein function with a negative predictive value of 80%. The p.R712G variant (also known as c.2134C>G), located in coding exon 19 of the POLE gene, results from a C to G substitution at nucleotide position 2134. The arginine at codon 712 is replaced by glycine, an amino acid with dissimilar properties. This amino acid position is conserved. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.
