This sequence change creates a premature translational stop signal (p.Tyr473*) in the POLE gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in POLE are known to be pathogenic (PMID: 23230001, 25948378, 30503519). This variant is present in population databases (rs369961557, gnomAD 0.0009%). This variant has not been reported in the literature in individuals affected with POLE-related conditions. ClinVar contains an entry for this variant (Variation ID: 484534). For these reasons, this variant has been classified as Pathogenic. -

The p.Y473* variant (also known as c.1419C>G), located in coding exon 14 of the POLE gene, results from a C to G substitution at nucleotide position 1419. This changes the amino acid from a tyrosine to a stop codon within coding exon 14. This variant was not reported in population based cohorts in the following databases: Database of Single Nucleotide Polymorphisms (dbSNP), NHLBI Exome Sequencing Project (ESP), and 1000 Genomes Project. In the ESP, this variant was not observed in 6503 samples (13006 alleles) with coverage at this position. To date, this alteration has been detected with an allele frequency of approximately 0.001% (greater than 80000 alleles tested) in our clinical cohort. This amino acid position is highly conserved in available vertebrate species. As neither this specific alteration nor loss of function as a mechanism of pathogenicity have been well-described in the POLE gene, the clinical significance of this variant remains unknown (ACMG Standards and guidelines for the interpretation of sequence variants. Genet Med. 2015 May;17(5):405-23). -
