Variant summary
Our verdict is Uncertain significance. The variant received 3 ACMG points: 3P and 0B. PM1PP3
The NM_006231.4(POLE):c.1378G>A(p.Val460Met) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000621 in 1,611,092 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. V460A) has been classified as Uncertain significance.

This variant is classified as a variant of uncertain significance as there is insufficient evidence to determine its impact on protein function and/or cancer risk. -
not provided Uncertain:2
This sequence change replaces valine, which is neutral and non-polar, with methionine, which is neutral and non-polar, at codon 460 of the POLE protein (p.Val460Met). This variant is present in population databases (rs753586583, gnomAD 0.003%). This missense change has been observed in individual(s) with clinical features of POLE-related conditions (PMID: 26251183). ClinVar contains an entry for this variant (Variation ID: 405748). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt POLE protein function with a positive predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
Not observed at significant frequency in large population cohorts (Lek et al., 2016); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Observed in at least one individual with cutaneous melanoma (Aoude 2015); This variant is associated with the following publications: (PMID: 26251183, 27535533, 20951805) -
Hereditary cancer-predisposing syndrome Uncertain:1
The p.V460M variant (also known as c.1378G>A), located in coding exon 14 of the POLE gene, results from a G to A substitution at nucleotide position 1378. The valine at codon 460 is replaced by methionine, an amino acid with highly similar properties. This alteration has been detected in an individual diagnosed with cutaneous melanoma at age 29 and her father who was diagnosed with prostate cancer, colorectal cancer, and acute myeloid leukemia; additional relatives in this family were diagnosed with colorectal, esophageal, and prostate cancers (Aoude LG et al. Fam. Cancer, 2015 Dec;14:621-8). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
